HOSPICE SERVICES

Hospice Care, provides a special form of care for people with life-limiting illnesses. Hopice cares for adult and pediatric patients with a wide range of life-limiting illnesses, including cancer, stroke, heart disease, lung disease, liver disease, kidney disease, multiple sclerosis, ALS, Alzheimer’s and AIDS.

What Does HOSPICE Do?

When medical treatments can no longer cure a disease, Hospice Care has a team of hospice professionals that still can do a great deal to control pain, reduce anxiety and offer needed spiritual and emotional support to patients and their families. Hospice services are provided wherever people with life-limiting illness reside and could benefit from hospice care:

  • Independent Living Communities
  • Patients’ homes
  • Dedicated inpatient hospice units
  • Hospitals, nursing homes, assisted living communities and residential care facilities
  • Hopice services are coordinated with the patient’s physician and provided during regularly scheduled visits by hospice team members to manage the patient’s care. If a crisis should arise, Hospice is able to respond 24 hours a day, seven days a week. Hospice Care also provides a continuum of other palliative services to complement traditional hospice care, helping patients and families clarify their care goals and achieve comfort and symptom relief.

    When everything is changing, Hospice Care helps things stay familiar. We help patients stay home whenever possible—whether home is a private residence, an assisted living or long term care facility, or even a hospital room. We work with patients’ personal physicians, so patients keep the doctors they know and trust. No matter where care takes place, we coordinate closely with all caregiver’s, spiritual advisors, families and friends to ensure the patients’ optimal well-being.
